These are partial reports have been uploaded (See Doc G) for periods around the time we believe Elvie was in this facility. The reports show a history of sub-standard care. Elvie was rescued from the SBTCC on January 24, 2022. At this time, it is not known how long she was in SBTCC facility before being rescued.
Under Health Inspections rating - SBTCC is shown as "Not Available" with the Reference No. of 18 which indicates SBTCC is not rated due to a history of serious quality issues, and it is included in the Special Focus Facility Program.
The Penalties section references the following note: "This facility is not rated due to a history of serious quality issues. This nursing home is subject to more frequent inspections, escalating penalties, and potential termination from Medicare and Medicaid as part of the Special Focus Facility Program."

These are reports that should have been available for the family to review prior to placing Elvie in the facility. These reports alone reveal a history of sub-standard care.
There are much better facilities available - why was Elvie put in yet another sub-standard facility by her family?
